Lighthouse Pharmaceuticals presented new analyses from the Phase 2/3 GAIN trial at AAIC 2026 showing that atuzaginstat, a gingipain inhibitor targeting p. gingivalis, was associated with improvements in several neuropsychiatric symptoms among p. gingivalis-positive patients with mild-to-moderate Alzheimer’s disease.
The company previously reported that atuzaginstat slowed cognitive decline by 57% (p=0.02) on the ADAS-Cog11 in a prespecified subgroup of p. gingivalis-positive patients.
New Neuropsychiatric Inventory subdomain analyses reportedly showed significant benefits in agitation/aggression, disinhibition, elation/euphoria and apathy after 48 weeks versus placebo, accompanied by significant reductions in caregiver distress; no significant benefit was observed in p. gingivalis-negative patients.
Lighthouse is advancing LHP588, a next-generation gingipain inhibitor with once-daily dosing and favorable Phase 1 safety results, and is conducting the Phase 2 SPRING trial in approximately 300 p. gingivalis-positive Alzheimer’s patients in the United States, which includes assessments of agitation, aggression, cognition and function.
