Therini Bio has dosed the first patients in a Phase 1b multiple ascending dose trial of THN391 for diabetic macular edema. The study will enroll three dose cohorts with each patient receiving three monthly intravitreal injections and will assess safety and biological activity using retinal central subfield thickness, visual acuity and exploratory biomarkers; initial data are expected in Q4 2026.

THN391 is described by the company as a high-affinity humanized monoclonal antibody designed to selectively block fibrin’s inflammatory epitope while preserving coagulation. The company cited a paper in the Journal of Neuroinflammation (Kantor et al., 2026) that reported THN391 matched VEGF antagonists in containing leakage in preclinical models.

Therini Bio has selected THN622 as its lead fibrin/VEGF bispecific candidate. The company claims the dual mechanism could combine VEGF blockade with fibrin inhibition to increase efficacy, response rates and durability, and to potentially prevent fibrosis in retinal neurodegenerative conditions.

Therini Bio is a clinical-stage biotechnology firm developing fibrin-targeting immunotherapies for vascular-mediated neuroinflammation and related diseases, including Alzheimer’s disease and diabetic macular edema.
